Jennifer Jacobs, PsyM, QMHS, CDCA
Executive Director
Before being appointed Executive Director in 2023, Ms. Jennifer Jacobs built a deep and comprehensive understanding of Leading Families Home (LFH) programs and operations through her prior role as Community Programs Director. In that capacity, she was responsible for researching and writing approximately 95% of LFH’s programmatic and operational grants, while ensuring program excellence, rigorous evaluation, budget compliance, and adherence to CARF accreditation and OHIOMAS standards.
Ms. Jacobs currently manages over $3 million in federal and state grants, maintaining strict oversight of compliance, quality, and performance outcomes. She oversees all facets of LFH’s operations, including Emergency Family Shelters, HUD-funded programming such as Rapid Rehousing (RRH) and Permanent Supportive Housing (PSH), as well as LFH’s Behavioral Health and Substance Use Disorder (SUD) services across all three campuses.
In addition to her administrative leadership, Ms. Jacobs has developed and implemented curricula utilized by multiple agencies and led the creation of LFH’s Strategic Business Planning Process, guiding program expansion into new markets and service areas.
Prior to her leadership at LFH, Ms. Jacobs served as Chief Executive Officer for the Providence Center for Social and Economic Empowerment. She has also served as an instructor for a variety of community-based education programs, including GED, Life skills, Anger Management, Victims of Domestic Violence, Batterers Intervention Program, Parenting-Plus, and Workplace Violence Prevention.
Ms. Jacobs is deeply committed to serving at-risk and unhoused individuals, and she consistently demonstrates an ability to foster positive, compassionate relationships with both participants and colleagues. Reflecting on her role as Executive Director, she shares:
